INSTRUCTIONS

PERSONAL DATA. Birth date is required. No personal data is ever sold or made available to other companies.

TYPE OF BICYCLE. Tandems, recumbents and tandem recumbents take up more space on sag wagons, so we need to know the count to ensure sufficient sag support.

INDICATE RIDE STATUS. Tell us what you are doing on the ride by filling in the payment amount for your selection.

“Full Tour” Participants are those riding from Hodgenville, KY to Springfield, IL from August 16-23, 2008.

“Non-rider” Participants are friends and spouses of riders who wish to join the tour for meals and events.

TOUR DE LINCOLN JERSEY. Check size box and fill in the amount. Jerseys are produced by Primal Wear. Jerseys will be mailed in July.

TOTAL AMOUNT ENCLOSED. Be sure to check your math. Applications with errors may be returned. Make check or money order payable to “Tour de Lincoln”.

AGREEMENT, WAIVER AND RELEASE OF LIABILITY. EVERY participant must complete a waiver agreement. This agreement must have the original and authentic signature of the participant.
